• NDIS information — NDIS number, plan details, support goals and funded supports
• Health information — medical conditions, medications, mobility needs, dietary requirements (only what is necessary to deliver safe support)
• Next-of-kin or nominee details
• Employment information for job applicants — resume, qualifications, references, working with children/vulnerable people checks

• To deliver NDIS supports as outlined in your service agreement and plan
• To roster suitable support workers and coordinate care
• To communicate with you, your family or nominees

We do not sell your information. We may share it with:

• Authorised support workers and contractors providing services to you
• The NDIA, plan managers and support coordinators where relevant

Your information is stored in secure systems hosted in Australia where practicable. We use access controls, encryption in transit, and staff training to protect personal and health information. Records are retained for the period required by NDIS rules and Australian law.

You can request access to or correction of the personal information we hold about you by contacting us using the details below. We will respond within a reasonable time, in line with the APPs.
